General FYSA Information
April 14: On Line registration for fall travel soccer begins. Please go to
the following site to register before May 2 so as not to be charged a late
fee of $25/player:

After you have completed the registration process, please click "Download Registration Forms" below.
May 2:
Late fee goes into effect. The following forms and any fees are due:
BIRTH CERTIFICATE: If your child is new to FYSA, please fax or mail a copy
of your child¹s birth certificate for Martha Bohrmann (fax:
781-4121/mailing address: 236 Foreside Rd., Falmouth, ME 04105).
CODE OF CONDUCT FORM must be signed by player and child and either faxed or
mailed to Martha Bohrmann (fax: 781-4121/mailing address: 236 Foreside
Rd., Falmouth, ME 04105).
UNIFORM ORDER FORM AND (OPTIONAL) WARM UP FORM must be mailed to Juli
Bazinet (15 Slocum Dr., Falmouth, ME 04105).
EVALUATIONS will take place for the U12, U13 and U14 players ONLY on the
following dates at the Portland Sports Complex (behind Joker¹s on Warren Ave
in Portland. Directions can be found at www.portland sportscomplex.com).
Make sure your child wears cleats or indoor soccer shoes and shin guards and
brings a soccer ball and water bottle.
May 5: 6:00-7:30 pm, U12 Boys and Girls
May 6: 6:00-7:30 pm, U14 Boys and Girls
May 7: 6:00-7:30 pm, U13 Boys and Girls
UNIFORMS:
If you are unsure of what size to order, sample uniforms and warm ups will
be available to try on during evaluations.
U12-U14 players may try on uniforms May 5, 6 or 7, 5:30-6:00 pm
U9- U11 players may try on uniforms May 5, 6 or 7, 6:30-7:30 pm.

On Team Selection and Cutting
FYSA makes every attempt to accommodate as many children as possible in its programs. But please be aware that state-mandated roster limits and other factors can force the club to make cuts at certain age groups in any given year. For more information, see the Tryouts/Team Selection section of this site.
Financial Aid
Financial aid is available for families in need. All requests are considered individually by the FYSA Board and all inquiries remain strictly confidential. Contact any Board member to inquire.
